You agree to allow members and affiliates of Brass Monkey Aerial Training Centre to take photos of you and use them for private and commercial use. Photos will be used on public platforms such as print advertisement, Facebook and news outlets.

I understand the acrobatic and circus arts can be dangerous. It is possible for me to injur myself minor or severely when in class at 95 Prospect St or publicly at any event associated with Brass Monkey Aerial Training Centre or the Canadian Pole Sport and Arts Federation. I understand and agree that I am SOLELY responsible for any injuries physical or mental that may occur due to my participation. I understand and agree that I am SOLELY responsible for any medical expenses or other expenses (such as loss of wages) associated with this injury. By checking yes below I agree and am releasing Brass Monkey Aerial Training Centre and it's affiliates (coaches, volunteers, other students, Ashley and carl wiggins) of any liability or responsibility.

By agreeing to the following terms I am also confirming I am the legal guardian and/or parent of the youth participant or participants named below. I agree to allow Brass Monkey and it's affiliates to take photographs and video of my child for promotional and resale purposes and I understand that I do not own rights to these images. I agree that pole and aerial circus training is s dangerous activity and sport. I understand this of these youth participants may be injured by participating. I release brass monkey Aerial Training Centre and it's coaches, affiliates and property of any legal or financial responsibility that may be associated with any injury/death physical or mental that may occur due to their participation.
